Background: Severe positive sagittal malalignment can potentially lead to shortening and contracture of the psoas joint capsule in a flexed spinopelvic position. The utilization bilateral release supplement spinal deformity correction same hospitalization was not reported literature.Case Presentation: A 66-year-old patient presented with 5-year history severe global (sagittal vertical axis 220 mm, 60° mismatch) that did improve on supine radiographs, modified Thomas test more than 30° flexion hips. three-stage operation utilizing posterior column osteotomies, anterior lumbar interbody fusion, releases performed.Conclusion: This report is first demonstrate safe substantial deformities associated hip one hospitalization.
